Main duties of the job

The Royal Berkshire Hospital has many specialities and as a rotational pharmacist you will experience all areas on a 3 monthly basis and be supported by clinical pharmacists and lead specialty pharmacists:

• Medical - Gastro-intestinal/Hepatic, Respiratory, Renal, Cardiology, Rheumatology.
• Surgical - General, Orthopaedic, Trauma, Urology, Head and Neck, Ophthalmology, Surgical admission unit.
• Paediatric & Maternity - Special baby unit, Paediatric - including T&O, oncology, acute medical. Maternity - Labour ward, ante-natal and post-natal wards.
• Elderly Care- 3 general wards, Hip frailty ward.
• Admissions wards - Acute admissions, Medical admissions, Short Stay Unit.
• Medicine Information - general enquires and Patient helpline.
• Aseptics.

The successful candidate would be an enthusiastic, innovative registered pharmacist who has recently completed their pre-registration pharmacist training, or a qualified pharmacist who would like to gain hospital pharmacy experience.

Interested in an exciting clinical leadership role working to further develop the Integrated Neighbourhood Team (INT) model of care for Long Term Conditions (LTC) within Lewisham Borough as part of the Lewisham Integrated Medicines Optimisation Service (LIMOS)?

We are looking for an enthusiastic senior pharmacist with strong established clinical skills to lead the clinical and operational delivery of the Lewisham INT programme and improve outcomes for patients with multiple long-term conditions in Lewisham.

INTs are a model of care designed to meet the holistic needs of the local population, working with professionals and as a local community to ensure people get the right care at the right time, from the right people, and to tackle health inequalities. This full-time post will be key in ensuring delivery of this model as part of the developing Neighbourhood approach to care. As the programme will focus on patients with three or more long term conditions around CVD, diabetes, CKD and AF if the first instance, experience of management of patients in these areas would be beneficial.

Main duties of the job

The Principal Clinical Pharmacist Prescriber will be responsible for the clinical leadership and management of the Lewisham INT for Long Term Conditions. Key aims of the role will be to provide strategic support to develop, integrate and evaluate this new model of care across the 4 Neighbourhoods / 7 PCNs in Lewisham, with managerial oversight to ensure effective functioning of the core team.

Job summary

Join Central London Community Healthcare NHS Trust as a Band 8a Primary Care Network Pharmacist, supporting Hammersmith & Fulham North PCN. You will work alongside our established pharmacy team to provide specialist medicines expertise and help improve safe, effective and person-centred use of medicines.

Key responsibilities include:

• Conduct structured medication reviews, including for people affected by inappropriate polypharmacy or overprescribing.
• Develop and deliver prescribing clinics supporting the management of long-term conditions.
• Provide specialist medicines advice to GPs, nurses, community pharmacists, care coordinators, carers, families and other health and social care professionals.
• Contribute to Trust-wide medicines management, including policies, patient safety initiatives, medicines alerts and professional training.
• Work across GP practices within Hammersmith & Fulham North PCN, including lone working and travel between sites.

Main duties of the job

We are looking for an experienced pharmacist with the clinical expertise and collaborative approach needed to deliver high-quality medicines optimisation in primary care.

You will need:

• Relevant post-registration experience in GP practice/primary care.
• The required postgraduate qualification(s), including independent prescribing, DPP and diabetes-related expertise.
• Strong knowledge of prescribing and medicines management in primary care.
• Sound clinical decision-making and problem-solving skills across a broad range of conditions.
• Effective communication and relationship-building skills when working with patients, carers and multidisciplinary health and social care teams.
• The ability to organise your workload, manage competing priorities and work effectively both independently and as part of a team.
• A compassionate, flexible and person-centred approach.

Job summary

Work collaboratively with colleagues within the Medicines Optimisation Team to provide specialist clinical pharmacist support for delivery of the ICB's medicines optimisation priorities. The role will take responsibility for defined areas of work, supporting safe, effective, equitable and cost-effective use of medicines across the system.

This position will lead and support defined medicines optimisation projects and service improvement activities, working with pharmacists, pharmacy technicians and wider colleagues to implement agreed priorities, standards and guidance.

This position will undertake patient-facing and patient-supporting activities to promote effective medicines use, including engaging in shared decision-making conversations with patients, carers and healthcare professionals where appropriate. Provide expertise to address both the public health and social care needs of patients, including lifestyle advice, service information, and help in tackling local health inequalities.

The postholder will take responsibility for defined clinical, governance and transformation workstreams, supporting decision-making, escalating risks and providing specialist pharmaceutical input to pathways and commissioned services.

Main duties of the job

The postholder will need to build and maintain constructive working relationships with internal and external stakeholders, including but not limited to:

• Internal ICB staff including managers,colleagues,and senior management as appropriate

• ICS partners, including portfolio leads, Local Authorities and Provider Collaboratives

• Non-NHS organisations

• Members of the public and service users as appropriate

• Matrix work across all other teams to address interdependencies and ensure alignment of mutual objectives.

• External consultancy, as required.
