Job summary

This is a pivotal role for a Senior Clinical Pharmacist who wants to make a measurable difference to medicines safety, governance and patient care across out-of-hours and dental services. You will lead a focused review of medicines management arrangements, assessing how medicines are prescribed, supplied, stored, administered, reconciled, disposed of and escalated across the services. Working closely with the Lead Pharmacist and clinical, operational and governance colleagues, you will identify compliance gaps, patient-safety risks and opportunities to strengthen practice. Your specialist insight will directly inform improvements to clinical governance, medicines optimisation and assurance arrangements.

This is an opportunity to use your pharmacy expertise at a strategic level: reviewing high-risk medicines, controlled drugs, antimicrobial stewardship and emergency medicines; analysing incidents and audit findings; and translating evidence into clear, prioritised recommendations. You will produce a robust current-state assessment, risk and gap analysis, improvement plan and final report, helping shape safer, more consistent and more effective medicines management for patients

Main duties of the job

• Map and review end-to-end medicines management processes across out-of-hours and dental services, including prescribing, supply, storage, administration, reconciliation, disposal and escalation.
• Assess arrangements against legislation, regulatory and professional standards, licence conditions, organisational policies and governance requirements, identifying compliance gaps and good practice.
• Evaluate the effectiveness of existing controls and procedures, identifying patient-safety risks, unwarranted variation, duplication and opportunities to improve consistency and efficiency.
• Review the safe management of high-risk medicines, controlled drugs, emergency medicines and cold-chain products, escalating immediate safety or significant compliance concerns through appropriate governance routes.
• Assess prescribing quality, antimicrobial stewardship, medicines optimisation and access to pharmacy advice, identifying opportunities to strengthen safe and effective practice.
• Review incidents, near misses, complaints, audits, policies, documentation and training arrangements to identify systemic risks, recurring themes and areas requiring improvement.
• Engage with the Lead Pharmacist, clinicians, operational teams and partners; produce a documented risk and gap analysis, prioritised recommendations, improvement plan, audit criteria and a final report for relevant governance forums.

Job summary

The Pharmacy Technician Apprenticeship provides fully funded training, nationally recognised qualification, and a salary while you learn.

After completing the programme, you can register with the General Pharmaceutical Council (GPhC) as a qualified Pharmacy Technician.

This role suits those interested in science, healthcare, and IT, and who want to make a positive impact.

Training is intensive and requires strongmotivation, commitment, and organisation.

You'll need to complete around 8-10hours of home study each week alongside your job.

Good communication, attention to detail, time management, and the ability to prioritise and meet deadlines are essential.

The role includes working evenings, weekends, and bank holidays.

Applicants must meet all apprenticeship eligibility criteria, including four GCSEs (or equivalent) at grades 4-9 / A-C* in English Language, Maths, and Science.

These must be clearly listed on the application form; those who do not meet the criteria will not be shortlisted.

Candidates who already hold a Level 3 (or higher) Pharmacy qualification are not eligible for apprenticeship funding.

Main duties of the job

Supervised duties will include:

• Labelling, dispensing and accuracy checking a variety of prescriptions using our electronic prescribing system.
• Learning how different medicines work and what they are used to treat.
• Ordering, receiving and maintaining pharmaceutical stock.
• Providing excellent customer care and maintaining confidentiality while dealing with patients, staff and the public.
• Counselling patients on how to use their medicines safely.
• Working directly with patients on wards and in clinics to ensure safe medicines use both while in hospital and on discharge.
• Completing patient medication histories onwards.

• To be the lead pharmacist for Urgent Care Frailty utilising specialist clinical knowledge to provide a high quality pharmaceutical service to patients

Substantiating clinical pharmacy recommendations at the highest level and against challenge from Consultants, where necessary, and effectively communicate any required actions to other medical and nursing staff.

Providing specialist clinical advice to other health care professionals on all pharmaceutical issues arising within the specialist area of cancer services. This will include the extrapolation of clinical and complex medicines information where data may be lacking and analysing, interpreting and comparing differing opinion or facts to determine the best prescribing strategy for an individual patient or patient group.

Assisting with the delivery of the WAHT pharmacy cancer services to achieve Trust and departmental medicines management targets including

Managing the accurate and timely dispensing and supply of medicines to cancer patients.

Adherence to NHS England standard operating procedures in relation to drugs supply through the Cancer Drugs Fund (CDF) and other commissioning arrangements.

Adherence to the Trust Formulary, unlicensed medicines policy and high-cost drug procedures.

Assisting with the creation of clinical guidelines relating to all areas of medicines use within the cancer services.

Assisting in the delivery of key pharmacy modernisation initiatives within cancer services and assisting in the assessment of the impact of such service development initiatives on medical and nursing staff within the post-holders area and the WAHT pharmacy department as a whole.

Medication History Taking, Provision of Patient Information and Counselling

Ensure that information and patient counselling systems are implemented within their specialist area according to patient need to ensure that patients obtain the most benefit from their medicines.

Set standards and manage the counselling of patients on their medicines within the Haematology directorate to improve concordance and knowledge of their therapy

Ensure all newly admitted patients are interviewed to construct an accurate medication history and assess compliance/concordance issues.

Review the medication history against the admission details and prescription chart

Pro-actively identify to medical staff any potential or actual medicines-related hospital admissions or associated issues and outline any required action to resolve identified discrepancies

Assess patients discharge medicines information requirements as early as possible in the admission stay to be able to prepare information on medicines for patients based on individual need.

Complete Patient Need Assessments to assess patients information requirements and suitability for delegation of counselling to a junior pharmacist or lead medicines management technician.

Counsel patients with highly complex needs such as a physical disability which may affect information requirements (sight or hearing), language difficulties, those with life-threatening or terminal diagnoses and highly distressed patients.

Managerial Responsibilities

Manage pharmacists as allocated including delivery of PDR and management of sickness based around trust policies.

Undertake the day to day supervision of pharmacists, technicians, assistants and students

Develop and maintain good internal communications within the department.

Advise and guide Consultants on proposed new medicines for use within the post-holders clinical area and assist with compiling new product assessments and evidence-based reviews for submission to the Area Prescribing Committee for formulary and funding approval

Identify cost containment programmes and introduce appropriate measures to keep the Haematology and Oncology Directorates drug expenditure within agreed budgets. Discuss and influence drug expenditure and its management with key medical staff, directorate managers and Modern Matrons within the post-holders specialist area.

Service Development and Business Planning

Develop formal links with the Clinical Director, Business Manager, Modern Matron and Ward managers in relation to the pharmaceutical needs.

Lead on key pharmacy modernisation initiatives within the Haematology team, ward and patients, and assess the impact of such service development initiatives on medical and nursing staff within the post-holders area and the pharmacy department as a whole.

Contribute to the senior clinical pharmacy team strategy group in planning and organising the delivery, standard and modernisation of clinical pharmacy services.

Research and Clinical Audit

Ensure all pharmaceutical aspects of research in the post-holders clinical area complies with relevant legislation

We are keen to recruit an enthusiastic, forward-thinking pharmacist to continue to lead and develop clinical pharmacy services within our Hammersmith and Fulham (H&F) MINT (Mental Health Integrated Network Teams) Services. Main duties of the job

The successful candidate will lead and develop the clinical pharmacy services to the H&F MINT teams and work together with the Advanced Specialist Pharmacists for Ealing MINT and Hounslow MINT, providing mutual support and cross-cover as necessary.

The MINT 8a pharmacists will support the Band 6 MMT EIS/CRS with clinical queries. Conversely, the Band 6 MMT EIS/CRS may assist the MINT 8a pharmacists with operational queries within MINT.

Job summary

We are seeking a highly motivated, enthusiastic and proactive Pharmacy Technician to join our busy and friendly Pharmacy Aseptic Services team on a 12-month fixed-term contract. The successful candidate will be a registered Pharmacy Technician with recent hospital pharmacy experience and a proven commitment to delivering safe, high-quality patient care.

The post is predominantly based within Preparative Services and involves the preparation, dispensing and checking of aseptic products, including systemic anti-cancer therapy and total parenteral nutrition, in accordance with Good Manufacturing Practice. The role also supports the Medicines Management Service provided to the Oncology Day Case Unit and Georgina Ward.

You will work as part of a dedicated multidisciplinary team, supporting service delivery, training colleagues and maintaining the highest standards of quality and patient safety. Experience in aseptic services and current aseptic validation are essential.

Working hours are 37.5 hours per week on a rotational basis between 7.30am and 5.20pm, Monday to Friday, with participation in late night, weekend and bank holiday rotas.

Main duties of the job

Supervision and Training: Support the supervision and daily training of other pharmacy staff, ensuring the highest standards of service delivery while observing good manufacturing practice.

Aseptic Manufacturing and Preparation: Undertake aseptic manufacture and preparation processes in sterile conditions using specialist equipment,ensuring compliance with safety and quality standards.

Documentation Preparation: Prepare all necessary documentation for aseptic processing, ensuring accuracy and regulatory compliance.

Handling Queries and Complex Issues: Address queries and resolve complex issues related to appointment schedules and prescribing treatments from hospital staff and patients, ensuring clear communication and efficient service.

Accredited Checking Technician:Undertake final accuracy checking within Aseptic Services (ACT). If not already, work towards or becoming a /or obtain a Pre and In-process checking qualification for aseptic services.

Stock Management: Perform stock management duties, ensuring efficient use and replenishment of aseptic supplies to maintain service continuity.

Quality Management: Contribute to the ongoing development and maintenance of quality management systems within the Aseptic Unit.

Medicines Management Service: Support the provision of Medicines Management Services to the Oncology Day Case Unit and Georgina Ward, ensuring safe and effective use of medicines for patients.
