Working closely with Pharmacists, Doctors, Nurses and the wider multidisciplinary team, you will help ensure patients receive the right medicines, information and support throughout their treatment. Main duties of the job

Main duties of the job

You will be based at St Ann's Hospital mainly working within the pharmacy dispensary , as the Senior Pharmacy Support Assistant . Support the department with house keeping duties. Performing ward top-up duties, this requires visiting in-patient wards, currently only St Ann's but this maybe expanded to the wider Trust.

Supporting other trainees within pharmacy when confident to do so. As part of the Trusts induction programme you will be expected to take part in breakaway training.

The pharmacy department at Lewisham & Greenwich NHS Trust is looking for a passionate and driven pharmacist to join our thriving Cancer & Technical Services team. As a senior member, you will gain valuable experience in the clinical management of Oncology and Haematology patients, as well as Technical Services. Main duties of the job

SACT day units (oncology & haem-onc outpatients) liaising directly with the medical and nursing team to deliver a clinical service to patients attending the hospital as day case treatments

Haemato-oncology wards (inpatients) - We currently have inpatient beds at both sites where you will provide clinical ward duties as well as participating in the multi-disciplinary haematology ward rounds working with the medical and nursing teams

Adult parenteral nutrition - We participate in parenteral nutrition ward rounds liaising with both dieticians and the medical team to provide parenteral nutrition to our adult patients. You will gain experience in the provision of clinical parenteral nutrition

Community hospice and palliative care - We work in partnership with the Greenwich and Bexley Community Hospice providing them with a clinical pharmacist service. The role provides an opportunity to gain experience in palliative care and management of patients liaising directly with our Consultant Palliative Care clinicians and specialist palliative care nurses

Job summary

We are seeking a pharmacist with previous experience of working in primary care to develop and manage Medicines Optimisation services within general practice. INTs are a model of care designed to meet the holistic needs of the local population, working with professionals and as a local community to ensure people get the right care at the right time, from the right people, and to tackle health inequalities. This full-time post will be key in ensuring delivery of this model as part of the developing Neighbourhood approach to care. As the programme will focus on patients with three or more long term conditions around CVD, diabetes, CKD and AF if the first instance, experience of management of patients in these areas would be beneficial.

Main duties of the job

The Principal Clinical Pharmacist Prescriber will be responsible for the clinical leadership and management of the Lewisham INT for Long Term Conditions. Key aims of the role will be to provide strategic support to develop, integrate and evaluate this new model of care across the 4 Neighbourhoods / 7 PCNs in Lewisham, with managerial oversight to ensure effective functioning of the core team.

The post holder will also provide clinical care to conduct patient facing multi-LTC reviews and optimise medicines management for patients, across one PCN and/or Neighbourhood footprint. Building strong relationships across system partners, supporting workforce development to deliver the new model of care and close working and communication with members of the core INT and wider teams across both health and social, and primary and secondary care settings will be key.

KIWY PCN are delighted to offer this exciting post of Clinical Pharmacist under the Additional Roles Reimbursement Scheme. You will be based full-time within Woodstock Surgery joining the existing 4 pharmacists and pharmacy technician across our Primary Care Network. Birmingham Women's Hospital is one of two dedicated women's hospitals in the UK, with the busiest single site maternity unit, delivering more than 8,200 babies a year offering a full range of gynaecological, maternity and neonatal care.
