Main duties of the job

• Provide an advanced clinical pharmacy service to the designated clinical area, taking professional responsibility for the pharmaceutical care of patients with complex medicines needs, including prescription review, medicines reconciliation, adverse drug reaction reporting, therapeutic drug monitoring and individualisation of drug therapy.
• Act as the first point of contact for complex medicines queries from medical, nursing and allied health professional staff, giving expert advice where evidence is limited or conflicting, and escalating where appropriate.
• Undertake regular dispensary sessions, including clinical screening and final checking of prescriptions, or homecare prescription screening, ensuring homecare prescriptions are clinically appropriate, meet commissioning and Blueteq requirements where applicable, and are processed in a timely way.

Job summary

This post is a fixed contract is designed for newly qualified GPHC registrants to gain hands on clinical experience across mental health and community hospitals.

The post will initially be based at St Ann's but providing clinical service to other hospital sites will follow on experience.

This is a part time post of 18.75 hours per week on a fixed term contract.

Main duties of the job

To be part of the Dorset HealthCare Medicines Team (DHMT) providing clinical pharmacy and medicines optimisation support to patients in the inpatient and patient home setting.

Provide clinical pharmacy support to community and mental health inpatient beds within Dorset HealthCare working closely with the clinicians covering these beds.

Using pharmaceutical expertise, to clinically review all prescription charts following admission of patients to the post-holder's allocated wards/units, ensuring patients are prescribed safe and effective medication. This includes monitoring all prescription charts (paper and electronic) to ensure safe, legal and evidence based prescribing takes place - and safe accurate administration - including clinical screening for drug interactions, overdoses or inappropriate prescribing e.g. renal, liver, cardiac impaired or pregnant patients.

To support the pharmacy technicians and ward/unit staff to ensure that adequate ward/unit pharmacy stock levels are maintained on allocated wards and to regularly review and adjust levels if necessary depending on prescribing trends.

To compile a drug history where possible from patients admitted to Trust services to ensure continuity of their medication and records relevant information in order to facilitate the discharge process.

Job Purpose

The Chief Pharmacist and Clinical Director for Pharmacy provides strategic, professional, clinical and operational leadership for Pharmacy and Medicines Management services across the Trust. The postholder is accountable for ensuring the safe, effective and cost-efficient use of medicines across inpatient, outpatient and community services, while contributing to organisational performance, patient safety, quality improvement and financial sustainability.

Job summary

This is an exciting opportunity to join UHDB in a newly created role.Are you an experienced clinical pharmacist passionate about improving care for people living with HIV and promoting the safe, effective use of antimicrobials? UHDB is seeking two Advanced Pharmacists - HIV and Antimicrobials to lead and develop our specialist HIV pharmacy service.

You will be an integral member of the multidisciplinary HIV team, working with consultants in genitourinary medicine, sexual health and microbiology, specialist nurses, pharmacists and wider clinical colleagues to provide high-quality, patient-centred care.

You will provide advanced clinical leadership for HIV medicines and support antimicrobial stewardship across the Trust. Responsibilities include medicines reconciliation, verification of antiretroviral therapy, and management of interactions, adverse effects and adherence concerns. You will undertake pharmacist-led reviews and independent prescribing within an agreed scope, using established multidisciplinary processes to strengthen treatment continuity and medicines safety.

This is an opportunity to shape HIV pharmacy provision at UHDB, improve transitions of care, build workforce capability and use NHS resources effectively. You will support readiness for national requirements, using audit, quality measures and financial reporting to demonstrate safety, compliance, patient benefit and value.

This full-time role is 37.5 hours per week and requires travel across UHDB sites.

• Provide proactive review for people living with HIV who are admitted to hospital.
• Identify, assess and manage drug interactions, contraindications, dose adjustments and adverse effects, using recognised specialist resources and clearly documented escalation pathways.
• Clinically screen and verify HIV prescriptions before supply, ensuring compliance with formulary, funding, reimbursement and governance requirements; arrange MDT or senior review for complex, high-risk, non-standard or restricted prescribing decisions.

Clissold Park PCN is seeking a Pharmacy Technician to work under the supervision of the Clinical Lead and to ensure the safe, accurate and timely supply of prescribed medication to patients. In addition, they will provide technical and administrative support to the Clinical Pharmacists and Clinicians as well as support practice primary care teams with queries around medication from patients; including face to face and telephone support to patients, primary and community staff.

To support the PCN Clinical Pharmacists in the delivery of clinical pharmacy services;

To perform medicines improvement work to maximise safe, cost-effective best practice in prescribing under the supervision of the GP or Clinical Pharmacist;

Job summary

The Pharmacy Department is seeking a motivated and ambitious pharmacist with a strong interest in oncology and haematology to join our Cancer Services team.

Based primarily at Kent & Canterbury Hospital, the post holder will provide specialist clinical pharmacy support across all three chemotherapy day units, the mobile chemotherapy unit, the inpatient haematology ward and the aseptic services unit.

This is an excellent opportunity to develop expertise in systemic anticancer therapy within a supportive and experienced multidisciplinary team. This role Provides leadership and management of our Pharmacy Aseptic Services Unit in accordance with current legalisation, Pharmacy, Trust, local and National Standards.

The unit produces over 3000 items a month mixed between ready to use and named patient dispensed and is in the process of a planned refurbishment as part of a longer term process of replacement.

It has a highly dedicated team and is supported by an in house Quality Assurance/Control service as well as extensive clinical pharmacy and clinical trails service

We are seeking a dedicated individual to play a key role in providing safe, high-quality medicines management, with a particular focus on supporting patients with mental health needs and ensuring robust monitoring of antipsychotic medications.This role combines both dispensary-based duties and clinical patient-facing services, offering variety and professional development in a complex and rewarding environment.Main Responsibilities:Dispensary & Medicines Management: Ensure the safe, timely, and secure supply of medications to prison wings. Oversee the safe transport and handling of medicines, adhering to Controlled Drug regulations. Provide clinical supervision to pharmacy technicians and support staff. Maintain the day-to-day operation of the prison dispensary.Clinical Duties: Conduct medication reviews and develop pharmaceutical care plans. Conduct comprehensive yearly audits focusing on compliance and best practices for Controlled Drugs, antibiotic usage, and the application of Patient Group Directions (PGDs). Provide structured medication reviews (SMRs) for eligible patients. Participate in clinics, offering advice on prescribed and over-the-counter medications. Manage minor ailments and common conditions within your clinical competency. Support patients in making the best use of their medicines through optimisation initiatives. Administer medications on the wing, depending on the business needsPrescribing & Monitoring: Clinically screen prescriptions to ensure accuracy, legality, safety, and effectiveness.
