HMP Wymott has capacity of around 1,100, and has a varied and complex population.

In accordance with the objectives set by the RFL Group Chief Pharmacist and Senior Operations Managers for CDC and Imaging, in line with medicines legislation to co-ordinate and develop the provision of high quality pharmacy services to patients visiting the services. An exciting opportunity has arisen for a full-time forward thinking, motivated and experienced aClinical Pharmacist to join our North Connect PCN in Northwood on a permanent contract.

The role will be based at Carepoint Practice.

The successful candidate will work at Carepoint Practice developing this role closely supported by the GPs. They will be part of a group of Clinical Pharmacists under the guidance and supervision of a delegated clinical supervisor.

We are looking for an enthusiastic Pharmacy Technician to join our Acute Medicine team.Oxford Centre for Enablement, a purpose-built rehabilitation centre on the Nuffield Orthopaedic Centre (NOC) site in Oxford. The OCE houses a number of multidisciplinary inpatient and outpatient services aimed at supporting people recovering from, and living with, disabling conditions. Job summary

We are looking to recruit an experienced and highly motivated Medicines Management Specialist Pharmacy Technician to join the pharmacy team at CPFT,working alongside a small but friendly, dedicated and experienced team of pharmacy staff.

Based at Cambridge Rehabilitation Unit (CRU), Brookfields Hospital in Cambridge, you will be responsible for the provision of the day-to-day clinical medicines management technician service to the wards at CRU, and in addition provide support to Welney Ward, at the Princess of Wales Hospital in Ely when needed.

This role will include undertaking patient medication histories, medicines reconciliation, assessment of patient's own medicines and maintaining the system of 'dispensing for discharge'.

Main duties of the job

The successful candidate will work closely with the specialist clinical pharmacists and multidisciplinary team to identify patients who have specific medicine management needs and plan to resolve these during their stay. You will promote rehabilitation and independence of patients, by supporting self-administration of medicines and providing medicines information and advice, helping to ensure medicines are used effectively, and improving awareness of medicines and their side effects. You will also support effective planning and organisation of medicines for discharge to enable a seamless transfer of care by liaising with appropriate healthcare professionals both within and outside the Trust.

The ideal candidate will have a flexible approach to needs of the service and have a strong teamwork ethic. They must be well organised and able to prioritise their own work, have good communication skills and be confident working independently, and as a fully integrated member of the multidisciplinary team. This role requires a strong sense of accountability, accompanied by the ability to work under pressure at times.
