We are looking for a highly motivated and experienced Healthcare Assistant/Facilitator who is passionate and interested in working in an Acute inpatient mental services for patients. You must be able to work flexibly and willing to adapt with the changing needs of our services. The post will require a high level of teamwork as well as a degree of autonomous working.

The inpatient is a dynamic, caring and compassionate environment. The team is highly motivated, focused and hardworking with a strong commitment to delivering high quality patient care. The therapeutic environment offers a broad range of interventions working co-productively with patients and cares to deliver an excellent service. There is a positive approach to reducing restrictive practice and promoting positive engagement across the unit.

The ward team is a multi - disciplinary mental health team that aims to provide a safe and effective clinical assessment and treatment service for patient requiring acute mental health treatment in hospital informally or under a Section of the Mental Health Act. The inpatient unit is situated at the Mental Health Unit, Claybrook Road, Hammersmith.

Main duties of the job

The post holder will work closely with the Ward manager and Registered Mental Health Nurses to provide safe, effective and high quality care for our adult patients with mental health needs. Job summary

An exciting opportunity has arisen for suitably qualified and experienced candidates who are registered with the NMC within our new Specialist Outreach Psychosis Service within the Borough of Ealing. Ealing is mobilising these new services bringing together the existing Community Rehabilitation teams alongside new investment to develop an assertive outreach pathway for service users with complex psychosis who need an assertive approach to engagement as part of their clinical presentation to provide care, treatment and support.

Successful candidates will show knowledge and experience of the clinical processes and treatments that support the care of people experiencing psychosis with complex needs. They will also demonstrate experience of working within a multi-disciplinary team setting; taking responsibility for assessing need and setting up robust care plans to support service user recovery. They will be able to take a flexible, robust and persistent approach to working alongside service users and their support networks to deliver care and get the greatest benefit from treatment.

We are seeking a passionate Clinical, Forensic or Counselling Psychologist to join the inpatient team at West London Forensic Service (WLFS). The post is 1.0 WTE for 1-year fixed term and will be based on a male low secure complex rehabilitation ward. Main duties of the job

You will be responsible for providing a high quality psychology service on a low secure complex rehabilitation ward. As such you will be able to utilise varied skills in assessment, formulation and evidence-based intervention, tailored to individual needs.

This is a role within the Green Paper for Transforming children and young people's mental health programme. The post holder will work with the qualified Occupational Therapists in the services delivering assessment and treatment sessions. The Occupational Therapy Assistant see patients on and off the ward. The sessions will generally be group sessions but could at times be one to one sessions.

Job summary

We are looking to recruit a highly motivated and flexible clinician with excellent clinical reasoning skills to join the Community Recovery Service.

The Community Recovery service is an integrated health and social care service for adults identified with uni and multi-disciplinary needs for recovery. The service will support independence by providing rehabilitation and reablement after an acute illness, injury or change in life circumstances. It aims to restore and maximise function or prevent deterioration in physical, psychological and social functioning through episodes of rehabilitation and reablement.

Therapists provide support and rehabilitation in the home.

All health and social care professionals within the Community Recovery Service will have a set of core skills that all staff in the team are able to deliver regardless of profession. This supports the single assessment process, maximises use of resources and minimises duplication.

In addition, team members will retain specialist skills and knowledge which will be utilised as required to support specific clinical pathways. Specialists will also support other team members' work to ensure fulfilment of relevant goals.

Main duties of the job

The successful candidate will act as a specialist clinician, participating as a member of the team while independently managing a specialist clinical caseload.

Manage and prioritise a professional caseload within the interdisciplinary service through efficient workload management, supervision and adjustment of service provision to meet changing priorities and ensure individual patients receive intervention timeously and appropriately.

They will provide high quality comprehensive Physiotherapy assessment and treatment to adults referred to the Hounslow Community Recovery Service.

Develop and deliver an individualised treatment programmes based on a problem solving and advanced clinical reasoning approach to the patients referred and monitor, evaluate and modify treatment to measure progress and ensure effectiveness of intervention using valid outcome measures.

Work closely with the multi-agency team to meet the needs of the patients and enable them to receive a comprehensive service.

Undertake all aspects of Physiotherapy care as an autonomous practitioner

(with support from a senior Physiotherapist & clinical lead).

Assess and document the patient's ability to give informed consent and engage in decision making that could be contentious requiring a high degree of motivation, negotiation or persuasive reassurance skills.

Working across GP practices and alongside West London NHS Trust's Mental Health Integrated Network Teams (MINT), you will provide holistic mental health assessments, risk assessments, brief interventions, care planning and support for adults with a range of mental health needs. You will play a key role in supporting people with serious and complex mental illness, facilitating access to appropriate services, and promoting recovery through a person-centred approach.
