Job summary
Are you an Occupational Therapist or Physiotherapist looking to develop your knowledge and skills working in the community?
We are a friendly, versatile and supportive community multi disciplinary team based within Solihull Hospital. Our patients all have a Solihull GP, and all have a risk or history of falls, often alongside complex vulnerabilities and health and social issues. Your role will include carrying out face to face holistic, multifactorial falls assessments within patients homes and care homes, before linking with the team, other community colleagues and the wider BSOL network to work together to reduce the future risk of falls for each patient. Our remit is large and we signpost and involve many different agencies to ensure that we address all identified risks as far as we can.
You will also be involved in triaging referrals, conducting telephone assessments, supporting students and junior staff, providing and attending in service training, and assisting with audit, research and leadership.
We are looking to increase our team of Therapists within the Falls Team and have a part time (33 hours a week) vacancy for a Band 6 OT or PT to join us! Please contact Helen Thomas for more information on 0121 424 5177

To take on a triage and co-ordinating role within the team.
To contribute to seven day working programmes across the organisation.
To contribute to the development of the Falls service including involvement in ongoing projects.

Band 6 OPAL Physiotherapist
University Hospitals Birmingham NHS Foundation Trust View all jobs
Posted 17 day(s) ago
Reference: 2383536
Job summary
An excellent opportunity has arisen for an experienced Physiotherapist looking to develop specialist skills in frailty, acute care and admission avoidance within a supportive and innovative multidisciplinary team. The Older Persons Assessment and Liaison (OPAL) Team at Good Hope Hospital are looking for an individual with a genuine passion for working with older people and delivering high-quality, patient-centred care.
The service supports admission avoidance, prevents deconditioning, and promotes independence through specialist assessment and treatment within the Emergency Department, Acute Medical Unit, and Medical Assessment Unit.
This is an exciting time to join the service as OPAL continues to expand, with the development of the Frailty Same Day Emergency Care (FSDEC) unit to provide timely coordinated care for those with urgent healthcare needs that cannot be managed within the community, but who do not require an overnight hospital admission.
A Home First model, OPAL is committed to supporting people to remain independent and return to their home wherever possible. Close partnership working with community services and wider healthcare providers, ensures patients receive the right care, in the right place, at the right time.
OPAL is a seven-day service and works collaboratively across disciplines to ensure patients receive timely physiotherapy input when it is needed most.
Main duties of the job
As a Band 6 Physiotherapist, you will use your clinical expertise, leadership skills and knowledge of frailty to assess, treat and support patients throughout their journey, enhancing both patient outcomes and experience. You will be expected to work autonomously, contribute to service development, support junior staff and students, and play an active role within the multidisciplinary team.
The role requires excellent communication and interpersonal skills, with the ability to manage sensitive and complex conversations with patients, relatives and carers. You will also be confident in coordinating onward referrals and signposting patients to appropriate community services and support pathways to facilitate safe and effective discharge planning.
About us
University Hospitals Birmingham NHS Foundation Trust strives to have an inclusive culture where everyone feels like they belong, can thrive, knows that they add value and feels valued. We do this by developing compassionate and culturally competent leaders, being values driven in all that we do and by creating a welcoming and inclusive workplace that thrives on the diversity of our people. For those staff with a disability, including physical disability, long term health condition, mental health or neurodiverse condition, this also means being committed to making reasonable adjustments needed for you to carry out your role.
