Job summary
To lead the delivery of SFC Connect in Westminster, acting as a front door to wellbeing, physical activity, and community participation. The role focuses on delivering high-quality, holistic assessments and occupational formulations, enabling individuals to access the right opportunities at the right time. Through early intervention and system navigation, the Occupational Therapist will reduce escalation into higher-level services and support people to engage in meaningful, life-enhancing activity in their local communities.
The Westminster Connect Service is a high-impact, preventative model embedded within Everyone Active provision. It provides a brief, structured Occupational Therapy assessment, followed by co produced action planning and short-term enabling support, connecting individuals to appropriate physical activity, community, and wellbeing opportunities. The service bridges the gap between clinical systems and community provision, ensuring timely, personalised, and accessible support.
Main duties of the job
Undertake comprehensive, holistic Occupational Therapy assessments, evaluating occupational performance, identity, routines, motivation and the impact of physical, psychological, cognitive, social and environmental factors on participation and wellbeing.
Apply specialist clinical reasoning to assess risk, identify occupational needs, determine pathway suitability and make appropriate onward referrals. Develop person-centred occupational formulations, collaborative goals and evidence-informed intervention plans to address barriers, promote independence and improve functional participation.
Deliver short-term therapeutic interventions incorporating activity-based approaches, graded engagement, behaviour change strategies, confidence building and self-management techniques. Utilise physical activity as a therapeutic intervention to enhance occupational performance, establish meaningful routines and support sustainable participation.
Work collaboratively with multidisciplinary professionals, primary care, social care and community partners to coordinate appropriate care, facilitate safe transitions and promote inclusive environments. Provide specialist clinical advice regarding complex needs, reasonable adjustments and accessibility.
Maintain accurate clinical records, monitor outcomes and contribute to service evaluation, quality improvement and evidence-based practice.

About us
Sport for Confidence CIC is an Occupational Therapy-led organisation that centres and prioritises the needs of people and communities furthest away from physical activity and sport. We work locally and nationally to narrow the gap in occupational, health and social outcomes, enabling people to participate in ways that are meaningful to their lives.
Our approach brings together the expertise of occupational therapists, the physical activity workforce and people with lived experience. It is person-first, occupation-focused and systems-driven, recognising the relationship between the person, their environment, occupations and participation.
Occupational therapists are central to our practice. This is occupational therapy delivered within community and physical activity settings. Occupational therapists use their professional reasoning and core occupational therapy skills to understand what matters to people, identify barriers to meaningful participation, adapt environments and opportunities, and enable people to engage in the occupations and everyday activities that support their health, wellbeing, independence and sense of belonging.
Our occupational therapists work within everyday community settings, including leisure centres, local communities and primary care. This place-based approach creates opportunities to work proactively and preventatively, address environmental and systemic barriers and support people earlier.
