Would you enjoy working with a small team of psychological professionals in a general hospital environment. We offer psychological assessment and intervention to people who have experienced a wide range of physical health conditions and are struggling with the physical, emotional and practical consequences of their diagnosis and treatment. We work with multidisciplinary colleagues directly with pain management provision or indirectly through supervision groups.

You would be working in a service provided by UHCW under a service level agreement into the George Eliot Hospital (GEH). We provide the general Clinical Health Psychological Service which accepts referrals from most clinical areas within the hospital, a paediatric diabetes post and a stroke rehabilitation post provided under this SLA. You will be part of Psychological Service at UHCW which delivers psychological care within UHCW, outpatients and Coventry community health provision. We are a welcoming team the successful applicant will work collaboratively with psychological colleagues across GEH and UHCW.

A full UK driving license is essential for this post. Please note that an international driving license is not accepted.

Main duties of the job

• To work as a Clinical/Counselling/Health Psychologist within the Severe Intestinal Failure (SIF) Psychology Team. To provide a psychological approach as part of an interdisciplinary team, which include Allied Health Professionals, Nursing and Medics, providing services to adults referred into the UHCW Severe Intestinal Failure (SIF) Service.
• To be responsible for managing a caseload and working with the wider multidisciplinary team to support the care needs of patients referred into SIF.
• To provide specialist evidenced-based psychological assessment and treatment of complex cases, requiring detailed analysis, formulation and interventions so as to maximise quality of life.
• To provide specialist advice and consultation to a range of multi-agency professionals and non-professional carers, and utilise research skills for audit, policy and service development.
• To work autonomously within the professional guidelines and overall framework of the Trust and team's policies and procedures.
• To follow defined care pathways and other relevant pathways as required.
• To assist in the quality and activity monitoring systems

Job summary

Pennine Care are looking for pharmacists to join our team and are offering a development role which aims to give the postholder a broad experience of mental health pharmacy as it is practised in both inpatient and community settings. The Mental Health Pharmacist - Community Services and Inpatients will be responsible for the provision and continued development of a clinical pharmacy service for inpatient and community services under the supervision of the senior pharmacist in the area.

The post holder will work with both inpatient and community teams ensuring a seamless care approach as well as through joint working with home-treatment and primary care pharmacy teams.

An opportunity has arisen for an enthusiastic and competent Practitioner Psychologist to join our Community Forensic Psychology Service. This is an exciting area of forensic services to work in with an emphasis on prevention and keeping people in the least restrictive settings. The post holder would join a well-established and well respected forensic team: NELFTs low-secure forensic unit, Morris ward, has previously ranked first in the UK and Ireland according the Quality Network for Forensic Mental Health Services (QNFMHS) and we remain proud of the services we offer.

Main duties of the job The post holder will provide assessment and evidence-based interventions to service users who typically present with complex mental health issues including psychosis, trauma histories and substance misuse, in addition to offending behaviour. Interventions will include individual, group, and family work. The post holder will work mainly with service users in the community but some work with inpatient service users and teams may be required. This post would suit someone with a strong interest in multidisciplinary working and working with wider systems including family and carers. The role will require creative andinnovativeinput including offering consultation to forensic and non-forensic colleagues. The post holder will provide leadership on quality improvement and clinical audit and well as managing some aspects of the Quality and Outcomes Framework and work closely with high risk drug monitoring.This role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver an excellent service within general practice.Main duties of the job Provide expertise in clinical medicines review and address public health and social needs ofpatients in GP practices Reduce inappropriate poly-pharmacy and wasteful prescribing through clinical medication review Reconcile medicines following hospital discharge and work with patients and communitypharmacists to ensure patients receive the medicines they need post discharge. Contribute to reductions in medicine related hospital admissions and readmissions bysupporting patients to get the best outcomes from their medicines and identifying andaddressing medicines related issues Interface with community and hospital pharmacy colleagues and develop referral processesbetween primary care professionals including the promotion of the repeat dispensing service Manage the repeat prescribing reauthorisation process by reviewing requests for repeat prescriptions and medicines reaching review dates Manage patients and health care professionals medicine queries Increase quality and safety of prescribing through mechanisms such as audit and PDSA cycles Manage practice formularies to improve the quality, safety and cost effectiveness of prescribing Implement drug withdrawals and alerts e.g. MHRA aimed at improving medicines safety Work with primary care professional and patients to implement NICE and other evidence based guidelines Run clinics where medicines are the main interventions e.g. anticoagulant monitoring clinics, dependence forming medication reviews. Contribute to multimorbidity reviews and multidisciplinary reviews Provide medicines information and training to Practice healthcare professionals and admin staff Review Pathology results for patients on known medicines and as part of medication review and request outstanding bloods where needed. Your role will impact on the patients medication journey, focusing on de-prescribing, improving quality of care, and reducing medicines related admissions. You will manage long term conditions, support hospital discharge prescribing arrangements, provide specific advice for those on polypharmacy, and support the practices to develop the clinical pharmacist role. You will support the prescription team and ensure pharmacists in the network, and other local networks, collaborate ideas and support each other.You will be confident, enthusiastic and adaptable to change, with excellent clinical, communication and ICT skills.You should exhibit good organisational skills and ability to communicate effectively with a wide range of healthcare professionals and other stakeholders including patients, families, and carers.You will be comfortable working independently and as part of multiple teams as described.
