Job summary
Are you an experienced mental health practitioner looking to develop your skills within a fast-paced and rewarding crisis service?
The Mental Health Single Point of Access (MH SPA) and NHS 111 Mental Health Service is seeking skilled and compassionate Band 5 Telecoach to join our growing team in Ealing
This opportunity is ideal for individuals with substantial experience working in mental health crisis, triage, assessment or psychologically informed support roles who are looking to further develop their practice within an innovative frontline service.
Working within a supportive multidisciplinary team, you will provide frontline mental health triage and crisis support across telephone, digital and face-to-face settings. You will undertake assessments, deliver psychologically informed interventions, support risk identification and contribute to ensuring individuals receive the right care, at the right time, in the right place.
This is a highly rewarding but emotionally demanding role requiring resilience, compassion, sound judgement and the ability to remain calm within high-pressure situations.
The service operates 24 hours a day, 7 days a week and post holders will be required to participate in a full rota including days, nights, weekends and bank holidays.
All new starters will undertake a structured induction programme and must be available to attend during standard weekday working hours. Annual leave is not ordinarily approved during the induction period
Main duties of the job
As a Band 5 Telecoach within the MH SPA and NHS 111 Mental Health Service, your responsibilities will include:
Undertaking mental health triage and assessment of individuals presenting in crisis via telephone, digital and face to face contact
Completing psychosocial and risk assessments to support safe and effective decision making
Delivering brief psychologically informed interventions to support emotional regulation, de-escalation and distress tolerance
Formulating immediate support plans and contributing to crisis care planning
Signposting and referring individuals to the most appropriate internal and external support pathways
Escalating high-risk, deteriorating or complex presentations to senior clinicians appropriately
Liaising with professionals and partner agencies including police, ambulance services, GPs and mental health teams
Maintaining accurate and defensible clinical documentation
Supporting peers, new starters and junior colleagues within the team
Participating in service improvement, audit and reflective practice activities
This role offers excellent development opportunities for practitioners seeking to strengthen their experience in crisis assessment, risk formulation and frontline mental health intervention.

Job summary
Thank you for your interest in this Band 6 Mental Health Practitioner position within our Ealing Community Rehabilitation Service (CRS). This is a full-time and permanent position and attracts 15% Outer London weighting. We welcome applications from Registered Mental Health/LD Nurses (registered with the NMC) and qualified Social Workers (registered with Social Work England) who are interested in working in complex psychosis in the community.
Mental health practitioners play a key role in dissolving the barriers that can exist between mental health and physical health, social care, voluntary and community social enterprise organisations and local communities, and primary and secondary care, to deliver integrated, personalised, place-based and well-coordinated care for adults - moving towards a 'whole system' of mental health care, which will provide timely access to support, care and treatment.
We need qualified, experienced, skilled and professionally registered practitioners, to support people with mental health needs to access mental health care when and where they need it, and to be able to move through the system easily.
What we are looking for are highly motivated and enthusiastic practitioners with a passion for psychosocial rehabilitation who are excited by service development. You will need experience at band 5 (or equivalent) and of working in the community to be able to step up to this band 6 role.
Main duties of the job
Depending on your professional background, this role may include clinical assessment and evidence-based interventions, coordination of mental and physical healthcare, relapse prevention and recovery-focused support, alongside the psycho-social interventions that help people manage their problems and move towards individualised recovery on their own terms - surrounded by their families, carers and social networks, and supported in their local community.
You will be an autonomous practitioner who will carry continuing responsibility for a defined caseload of service users experiencing complex psychosis.
You will support individuals to be able to contribute to and be participants in the communities that sustain them. A key part of the role will be liaison with Local Authority and ICB colleagues to ensure that any out-of-area and supported accommodation placements are suitable for the needs of the service user and that people are supported in the least restrictive environment possible.
Please consider applying if you are passionate about supporting people to live as independently as possible and are committed to a strength based and recovery focused approach to supporting people with complex psychosis.

Job summary
An exciting opportunity to help shape a new specialist community mental health service in Hounslow.
We are looking for an experienced and enthusiastic Band 7 Clinical Team Lead to join our newly formed Hounslow Intensive Outreach & Rehabilitation Team (HIORT).
HIORT brings together our established Community Rehabilitation (CRS) and Assertive Outreach (AO) pathways into one integrated multidisciplinary team. This is an important development for Hounslow and an opportunity to build on the strengths, expertise and experience of both services, while retaining the distinct clinical approach each pathway needs and creating a more joined up offer for people with complex and enduring mental health needs.
As our Clinical Team Lead, you will be right at the heart of this development.
Working alongside the Band 8a Team Manager, and our wider multidisciplinary colleagues, you will help bring the new team together and shape how we work in practice. You will provide visible, supportive clinical leadership across both pathways, line manage and develop staff, support effective caseload management and clinical decision making, and help us establish a positive, accountable and compassionate team culture.
Main duties of the job
This is a hands-on clinical leadership role. Alongside your leadership and first line management responsibilities, you will retain a small caseload of people with highly complex needs and work alongside practitioners when clinical situations require senior support. We want someone who enjoys being close to clinical practice, supporting staff and finding practical solutions when things are difficult.
There is a real opportunity to make your mark. As HIORT develops, you will help us strengthen our clinical standards, embed effective ways of working across rehabilitation and assertive outreach, improve the experience of people using our services and ensure that staff feel supported to deliver excellent care.
You will be a registered mental health professional with a strong background in community mental health and in working with people with severe and enduring mental health needs. You will bring sound clinical judgement, confidence in managing complexity and risk, and the ability to lead people with warmth, clarity and accountability.
If you are ready for your next leadership challenge and excited by the opportunity to help build and shape a developing service, we would really like to hear from you.
