We are seeking an enthusiastic and motivated Band 5 Speech and Language Therapist to join our supportive Paediatric Therapy Team. As a Team Sister within our Community Transfer of Care Hub, you will play a pivotal role in supporting patients to leave hospital safely and receive the right care in the right place at the right time.

Based at Gravesham Community Hospital, you will be part of a newly established and growing service that manages referrals into the Discharge to Assess pathways across Dartford, Gravesham, Swanley and Medway.

Working at the heart of system-wide discharge planning, you will help ensure patients experience smooth transitions from hospital to community services, assessment beds or their own homes, supporting our Home First approach wherever possible.

The service operates 7 days a week:

The Community Staff Nurse in the School Nurse Team is a registered general nurse working as part of the public health nursing team in providing preventative and supportive health care to children, young people and their families.

The post holder works with a public health focus under the supervision of a Specialist Community Public Health Nurse (SCPHN) with a defined population.

As aBand 6 Community Paediatric Physiotherapist, you will work as part of the Paediatric Therapy Team to autonomously manage a complex caseload of children with a wide range of varied and complex needs.
