The role involves providing ward-based medicines management services to wards within West Park Hospital and accuracy checking in the dispensary.

Main duties of the job

The successful candidate will provide technical aspects of the pharmacy medicines management and medicines optimisation services, working at West Park Hospital in Darlington.

You will work with the other pharmacy colleagues, in the dispensary and wards, to provide patients with an accurate and timely supply of medication from admission through to discharge.

You should be able to competently clinically check an array of medication from a variety of order forms.

You will be responsible for a range of technical duties in the dispensary to aid in the smooth and efficient running of the department and will assist the Clinical Pharmacists and Senior Pharmacy Technicians in the provision of a ward-based medicines optimisation service.

You will deliver the technical clinical pharmacy service and promote medicines optimisation across a variety of specialities under the direction of the Lead Pharmacy Technician, whilst always promoting a positive image of people with mental health conditions and learning disabilities.

Job summary

We are seeking a Clinical Pharmacist to join our our well-established, friendly team of pharmacists and pharmacy technicians who provide clinical support to mental health units, community hospitals and community teams across Hampshire. This role is for 30 hrs per week.

The post is based at The Orchards Mental Health Unit at St James Hospital in Portsmouth and the successful applicant will provide regular clinical pharmacist support here and at the OPMH inpatient wards in Gosport War Memorial Hospital. You will also work closely with the East Region (Porstmouth and South East) pharmacy team to provide clinical cover to Older Persons Mental Health and community rehabilitation wards.

Job summary

We are looking for a Clinical Pharmacist to join our team, working at Desford Medical Centre. Whilst this is a fixed term position, it is going to cover 2 separate periods of maternity leave, with a provisional 18 month duration, but potentially extending to 21 months.

The FTE salary for this position is

We are looking for a pharmacist with previous NHS general practice experience who is familiar with EMIS Web, Ardens and primary care clinical workflows, and is comfortable working with a high degree of professional independence.

Main duties of the job

Job summary

Hillton Primary Care Network in Bedford are seeking an enthusiastic clinical pharmacist, to work across the practices within the PCN on a 12 month maternity contract. This role is offered on a full-time basis for 37.5 hours per week and will commence in January 2027. You will play a full and active role within the PCN multidisciplinary team. This role is pivotal to improving the quality of care and operational efficiencies so require motivation and passion to deliver an excellent service within general practice. Are you a proactive, self-motivated pharmacy technician who is passionate about delivering high-quality patient care? BCHC is seeking an experienced and enthusiastic GPhC-registered Pharmacy Technician to join our Intermediate Care Pharmacy Team.

The post holder will have the opportunity to support the development and delivery of the team's medicines optimisation programme, helping to provide the highest quality of care for our patients. Working as part of a multidisciplinary team, you will support the management of medicines during transfers of care from other providers and contribute to systems that promote safer prescribing and the safe and secure handling of medicines. A thorough understanding of medicines management and medicines reconciliation following transfers between care settings is essential. You must be able to resolve identified issues and communicate effectively with patients, families and other healthcare professionals.

Main duties of the job

The post holder will primarily work across the Intermediate Care Units, alongside the clinical pharmacists. Our units are located across Birmingham, so the ability to travel between sites is essential.

Duties will include Level 2 medicines reconciliation, assessment of patients' own medicines, self-medication assessments and discharge counselling. The post holder will also practise as an Accredited Checking Pharmacy Technician (ACPT), both in the dispensary and on the units, checking rewritten medication charts to confirm that they have been transcribed correctly.

The position will be based at Basildon site, where you will be part of an innovative team, and be involved in many exciting projects to promote and deliver high quality clinical service to our patients.

Main duties of the job

• Providing specialist clinical pharmacy services to Obstetrics and Gynaecology, and Paediatric wards
• Working closely with the multidisciplinary team and taking a proactive and leading role in implementing evidence based practice
• Training of the pharmacy and multidisciplinary teams including Trainee, Foundation and Specialist pharmacists
• Management of medicine shortages, ensuring that risk assessments are completed where required.
• Deputise for the clinical lead pharmacist where necessary including governance and clinical meetings.
• Develop and implement new protocols and procedures, and assist in development of pharmaceutical aspects of policies.
• Proactive in taking part in research opportunities.

Job summary

Mount Farm Surgery is looking to welcome a qualified and experienced clinical pharmacist into our busy practice, to play a full and active role within our multi-disciplinary team. The role is offered on afull-time basis, Monday-Friday.

Main duties of the job

• Work as part of a multi-disciplinary team in a patient-facing role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas
• Be a prescriber, or completing training to become a prescriber, and work with and alongside the general practice team
• Be involved in the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with complex polypharmacy, especially the elderly, people in care homes, those with multiple co-morbidities (in particular frailty, COPD and asthma) and people with learning disabilities or autism (through STOMP Stop Over Medication Programme)
• Provide specialist expertise in the use of medicines whilst helping to address both the public health and social care needs of patients at the organisation and to help in tackling inequalities
• Develop relationships and work closely with other pharmacy professionals across the wider health and social care system

There may be, on occasion, a requirement to carry out other tasks, this will be dependent upon factors such as workload and staffing levels

Job summary

The SWLSTG Pharmacy Team has an exciting opportunity for an experienced Mental Health Pharmacist to join our friendly and innovative department, leading medicines optimisation services within our community mental health teams.

The post is Monday to Friday, 9:00am to 5:00pm, with no regular weekend, bank holiday or late duty commitments currently required.

The postholder will deliver specialist clinical pharmacy services aimed at reducing hospital admissions through medicines optimisation, improving patient and carer experience with prescribed medicines, enhancing physical health monitoring and management, and strengthening communication between secondary and primary care. Opportunities to develop and utilise Non-Medical Prescribing skills will be available.

You will be an integral member of a multidisciplinary team working across community mental health services. The role offers an exciting opportunity to influence and shape service development, supporting service users to optimise their medicines and achieve better health outcomes.

This role will primarily support community mental health services across the London Borough of Richmond upon Thames.

Main duties of the job

Deliver specialist medicines optimisation services to community mental health teams, promoting the safe, effective and person-centred use of medicines.

Work closely with psychiatrists, non-medical prescribers, care coordinators, GPs and the wider multidisciplinary team to optimise treatment and support recovery.

Undertake medication reviews and make recommendations to initiate, optimise, switch or discontinue medicines where appropriate.

Support the early identification and appropriate use of clozapine and long-acting injectable antipsychotics to improve outcomes and reduce the risk of relapse and hospital admission.

Promote relapse prevention through medicines optimisation, adherence support, patient education and proactive management of medication-related side effects.

Improve physical health outcomes through medicines optimisation, physical health monitoring and collaborative working with primary care colleagues.

Provide expert advice and education to service users, carers and healthcare professionals to support informed decision-making and medication adherence.

Contribute to service development, quality improvement, audit, research and education initiatives across the borough.

Help shape and improve prescribing practice through clinical leadership, evidence-based recommendations and collaborative working.
