Main duties of the job

• To be the lead pharmacist for Urgent Care Frailty utilising specialist clinical knowledge to provide a high quality pharmaceutical service to patients
• Support medicines optimisation of patients with frailty within the Emergency Department, the Older Persons Medicine and Frailty Same Day emergency Care (OSDEC) service, and the Acute Medical Unit.
• Play an integral role in the planning, development and delivery of the Trusts restructuring of patient pathways within frailty and elderly care. This will involve working with members of the MDT in order to provide excellent clinical care as well as developing the Trusts ability to meet the demands of an ageing population.
• Provide expert clinical knowledge and advice on frailty medicine to enable the Pharmacy department to continue to develop its links with local Primary Care Networks (PCNs), including expanding the opportunity to create integrated roles across the secondary care and primary care boundary.
• To integrate the de-prescribing tool for use on frailty and elderly care wards which enables pharmacists and medical teams to de-prescribe in a structured and evidenced based manner therefore addressing the issue of polypharmacy.
• Practice as a non-medical prescriber focusing on frailty in the Emergency Department, OSDEC and the Acute Medical Unit.
• To provide expert advice on pharmaceutical matters when carrying out clinical pharmacy services to wards

Job summary

• Act as Specialist Pharmacist Lead for Oncology Services, providing professional leadership and acting as the key pharmacy contact for service developments, supported by the Principal Pharmacist for Cancer Services.

Cestria Health Centre is seeking a motivated and patient focused Pharmacist to join our friendly and supportive primary care team under an ARRS funded role. An opportunity has arisen for an Advanced Specialist Pharmacist - HIV (Band 8a) to lead and deliver a highly specialist HIV pharmacy service within Blackpool Teaching Hospitals NHS Foundation Trust.

The post holder will provide expert pharmaceutical care for patients with HIV across outpatient and inpatient settings, working as a core member of the multidisciplinary HIV team. Main duties of the job

This is a rounded, hands-on Lead Clinical Pharmacist role combining clinical practice with leadership and management responsibility. The postholder will provide professional leadership, line management, supervision, teaching, training and development for the pharmacy team.

Working closely with our Pharmacy Clinical Informatics Team, clinical pharmacy colleagues and wider multidisciplinary teams, you will lead EPS implementation, support system optimisation and training, and help us realise the benefits of digital transformation for our patients and staff. You will also work closely with our Medicines Safety Pharmacist and Medication Safety Officer, supporting the delivery of our medicines safety priorities. This will include reviewing and analysing medicines-related incidents reported through Datix, identifying trends and opportunities for learning, coordinating audits and supporting improvement initiatives. A further part of your role will involve supporting controlled drugs governance across our wards and clinical areas, coordinating audits, monitoring compliance and working with clinical teams to address identified risks and improve practice. Main duties of the job

The Pharmacy Technician is an integral part of the general practice team, working within their professional boundaries.

This role will work under the supervision of the Senior Partner to ensure the safe, accurate and timely supply of prescribed medication to patients.

The Pharmacy Technician will provide technical and administrative support to the clinicians.

The Pharmacy Technician will support the practice clinical teams with queries regarding medication from patients.

The Pharmacy Technician will complete audits regarding patient safety in the prescribing of medication and present the information for sharing with the clinical team.

The Pharmacy Technician supports public health and screening programmes including immunisations and vaccination programmes whilst participating in quality improvement initiatives to enhance patient outcomes and reduce health inequalities.
