Job summary
Are you an experienced HCPC-registered Paramedic looking to develop your clinical skills ?
An exciting opportunity has arisen to join the REACT (Reactive Emergency Assessment Community Team) service within East Suffolk and North Essex NHS Foundation Trust (ESNEFT).
Working within our Urgent Community Response (UCR) pathway, you will play a vital role in providing rapid assessment, treatment and clinical decision-making for patients experiencing an acute deterioration in their own homes. As part of a multidisciplinary team, you will support people to remain safely at home wherever possible, preventing unnecessary hospital admissions and delivering care closer to home.
This is an exciting alternative to traditional ambulance practice, providing opportunities to manage complex patients, develop advanced assessment skills and work alongside therapists, nurses, advanced practitioners, social care professionals, consultants and GPs across the health and care system.
The post is available for up to 37.5 hours per week as part of a 7-day service operating across weekdays, weekends and bank holidays.
Proposed Interview date: 15th October 2026
Main duties of the job
Undertake comprehensive holistic assessments of patients presenting with acute illness, frailty, injury or deterioration within their own homes and community settings.
Use advanced clinical reasoning and autonomous decision-making skills to assess, diagnose, treat, refer or safely discharge patients.
Deliver urgent care interventions in line with the national 2-hour Urgent Community Response standard.
Work collaboratively with Virtual Ward, Frailty, Community Nursing, Therapy, General Practice, Social Care and Ambulance Service colleagues to ensure seamless patient care.
Support admission avoidance and facilitate safe alternatives to hospital attendance wherever clinically appropriate.
Provide expert clinical advice and support to colleagues across the multidisciplinary team.
Contribute to service development, quality improvement initiatives, audit and clinical governance activities.
Support education, supervision and development of junior staff and students.
As healthcare services continue to evolve, REACT is at the forefront of NHS transformation, delivering innovative admission avoidance pathways, Virtual Ward services and integrated community care. You will have the opportunity to contribute to service development while making a meaningful difference to patients and their families every day.

Job summary
An exciting opportunity has arisen for an experienced, motivated and proactive Paramedic to join our Reactive Emergency Assessment Community Team (REACT).
REACT is an innovative multidisciplinary community service within ESNEFT, bringing together Urgent Community Response (UCR), Hospital at Home, OPAT and Out of Hours pathways to deliver high-quality, patient-centred care closer to home. The service works closely with the East of England Ambulance Service, Hospital at Home, Integrated Neighbourhood Teams and acute services to support admission avoidance and deliver rapid intervention for patients experiencing acute deterioration in the community.
You'll play a key role in delivering the national 2-hour Urgent Community Response standard, supporting patients to remain safely in their own homes wherever appropriate and promoting our Home First philosophy.
This role offers the opportunity to influence the development of innovative community pathways, strengthen system-wide relationships and contribute to the ongoing transformation of urgent community care services across East Suffolk.
The role is available for up to 37.5 hours per week and includes participation in a 7-day service operating between 08:00 and 20:00, including weekends and bank holidays.
Proposed Interview date: 15th October 2026
Main duties of the job
Provide expert paramedic assessment, clinical decision-making and intervention for patients with acute illness, frailty and complex health needs within their own homes.

This is an exciting opportunity for a Paramedic looking to work at an advanced level, combining expert clinical practice with leadership, service development and multidisciplinary collaboration. You will work autonomously whilst providing clinical leadership across the service, supporting staff development, patient flow and operational delivery.
