Job summary
Make a lasting difference to patient care across surgery, critical care and the interface between services. We are seeking an experienced, forward-thinking clinical pharmacist to join us in this pivotal Band 8a role, combining advanced patient-facing practice with leadership in medicines safety, governance and service improvement.
This is an essential clinical role with a clear purpose: to assure the safe, effective and evidence-based use of medicines across surgical and critical care pathways. The post is predominantly patient-facing, with 0.6 WTE dedicated to surgical and critical care inpatient services and patient flow. The remaining 0.4 WTE will focus on vital interface and Patient Group Direction (PGD) responsibilities, ensuring staff receive timely, expert advice and support.
You will be an adaptable team player and a compassionate, credible leader who can influence practice across professional boundaries. You will communicate clearly, maintain exemplary clinical documentation and role-model the professional accountability expected at this level to colleagues across pharmacy and the wider multidisciplinary team.
If you are ready to combine advanced clinical practice with great teamwork, effective leadership and meaningful service improvement for our patients, we would be delighted to hear from you.
Main duties of the job
Provide direct patient care and support safe, effective clinical treatment across surgery, critical care and patient flow.
Optimise medicines use by guiding medical and non-medical prescribing practice and, where appropriate, independently prescribing or deprescribing.
Apply local, regional and national guidance, demonstrating excellent antimicrobial stewardship.
Use sound clinical judgement and evidence-based practice to manage medicines shortages that affect patient care.
Write and contribute to Standard Operating Procedures, policies and guidelines, identifying and addressing gaps in local practice.
Recognise medicines-related incidents and near misses, support timely resolution, and escalate or signpost concerns appropriately.
Provide expert formulary advice, review clinical guidelines and lead the review of PGDs for the safe supply or administration of prescription-only medicines in line with locally agreed policy.
You will support the Pharmacist Clinical Team Manager in the delivery of the Pharmacy Vision and Values Strategy together with the other Clinical Pharmacy Team Leaders.
About us
The Humber Health Partnership is one of the largest acute and community Partnership arrangements in the NHS, seeing well over one million patients every year and managing a budget of over £1.3 billion.
Made up of two Trusts - Northern Lincolnshire and Goole NHS Foundation Trust (NLAG) and Hull University Teaching Hospitals NHS Trust (HUTH) - our Partnership has significant ambitions and is committed to delivering world-class hospital and community services for the 1.65 million people we serve.
Together we employ nearly 20,000 staff. Our five main hospital sites are Diana, Princess of Wales Hospital, Scunthorpe General Hospital and Goole and District Hospital, for NLAG and Hull Royal Infirmary and Castle Hill Hospital for HUTH.
We believe that by developing a diverse, inclusive, innovative, skilled and caring workforce, we can deliver excellent care to our patients and a great future for our employees, our Partnership and our community.
We are committed to supporting your professional development through education and training opportunities. Established pharmacists who meet the remaining essential criteria for the post will be supported to undertake the independent prescribing qualification. You will also have opportunities to contribute to wider Trust groups, champion organisational and community wellbeing, and help shape the future of pharmacy services.

Job responsibilities
This is a full-time vacancy of 33 hours per week on nights. As a Senior Care Assistant at HC One, Kindness will be at the core of everything you do. It touches upon every aspect of our Dementia, Nursing, Residential and Specialist care homes. Every single person who works here understands that our residents are people just like them with their own stories to tell. And, by joining us, youll have a wonderful opportunity to give something back to those people. For you, that will mean planning and implementing the kind of care that will give our residents a real sense of security, dignity and independence. You will get to know our residents and their families helping to ensure we deliver truly personalised care plans. Leading a team of Care Assistants, you will be responsible for maintaining the highest standards of care and youll be a real influence on the rest of the team. Whether you are showing someone how to use a walking frame, helping someone at the end of their life to eat a meal or talking to a residents family on the phone, youll do so with a sense of kindness, above anything else. Assisting the Registered Nurse and Care Manager, you will be involved in providing essential physical and mental stimulation through a range of social and recreational activities, as well as administering medication. One of your key responsibilities as Senior Care Assistant is to maintain accurate records of the job you do and build strong relationships with the rest of your team. Ideally, youll have completed a Level 2 Diploma S/NVQ in Adult Social Care and are prepared to work towards Level 3 qualification in Adult Social Care or equivalent, a background in person centered care planning and knowledge of caring for people with dementia. Whats essential, though, is your brilliant positive energy and natural ability to get along with people. Youll know what good record keeping looks like and be able to manage your time effectively. And, whenever you communicate with a resident or co-worker, youll be truly considerate and kind.
