Leading a team of Care Assistants, you will be responsible for maintaining the highest standards of care and youll be a real influence on the rest of the team. Whether you are showing someone how to use a walking frame, helping someone at the end of their life to eat a meal or talking to a residents family on the phone, youll do so with a sense of kindness, above anything else. Assisting the Registered Nurse and Care Manager, you will be involved in providing essential physical and mental stimulation through a range of social and recreational activities, as well as administering medication. One of your key responsibilities as Senior Care Assistant is to maintain accurate records of the job you do and build strong relationships with the rest of your team. Ideally, youll have completed a Level 2 Diploma S/NVQ in Adult Social Care and are prepared to work towards Level 3 qualification in Adult Social Care or equivalent, a background in person centered care planning and knowledge of caring for people with dementia. Whats essential, though, is your brilliant positive energy and natural ability to get along with people. Youll know what good record keeping looks like and be able to manage your time effectively. And, whenever you communicate with a resident or co-worker, youll be truly considerate and kind.

Job summary
A Care Navigator is a very special person! They are the first port of call for patients and they respond to requests for GP appointments, made by patients either on the phone or in person. With training and support, they will triage the patient non-clinically, and signpost to the appropriate service or person, which may not necessarily be the GP. They will work in a reception hub alongside their colleagues (a team of 12) or on front desk reception.
We are looking for someone who is flexible enough to cover early starts (8am) and late finishes (6.30pm) across the week working around 30 hours per week including a Monday, Tuesday and Thursday at least.
Salary starts at £13.00 per hour with an increase after you pass probation to £13.50 per hour.
If you have applied previously please do not apply again.
Main duties of the job
This is a challenging but very satisfying role.
Patients rely on our care navigators to listen, advise and point them in the right direction. Often, patients themselves do not know what they need in terms of their health and it is up to a trained navigator to support them. Our team prides itself on the care and customer service we offer. Our access to appointments is excellent and prompt, but we rely on our navigators to ensure that the right person is seen at the right time by the right clinician (not necessarily the doctor!) This takes tact and diplomacy as well as patience.
About us
East Trees Health Centre is based in inner city Bristol. We are a training practice with a list size of 15000 patients.
Our practice team comprises of 4 partners, 7 salaried GPs, a paramedic, nurse practitioner, physio, podiatrist, dietitians, care co-ordinator, care navigators, administrators, nurses and pharmacy team. We are a very friendly, close knit team and have regular staff and team meetings to ensure communication is consistent.

Director Strategic Commissioning (Client Groups/Community Based Care)
NHS South East London Integrated Care Board View all jobs
Posted 3 day(s) ago
Reference: 2434955
Job summary
We are looking for an exceptional Director of Strategic Commissioning to join South East and South West London ICBs working together as an Integrated Care Board cluster. This role will be pivotal for the clustered ICBs as they refocus on strategic commissioning for a population of 3.8 million people with a commissioning budget of £4.75 billion for South East London and £4.3 billion for South West London.
The successful candidate must have the experience and capability to lead and inspire others to seize the opportunities of a period of transformation for ICBs. They will need the determination and discipline to make sure strategic and operational plans address local need, drive out inequality, deliver value for money and secure improved outcomes for local residents across our boroughs. In doing so they will have the vision and ambition to realise the potential of neighbourhood working, population health management and digital technology, and will align the commissioning process to deliver the Governments three strategic shifts: from treatment to prevention, from hospital to community, and from analogue to digital.
We would love to hear from candidates with commissioning expertise, a proven track record of successful strategic leadership, and who are excited by the possibilities of this complex and challenging role across the second largest ICB footprint in the country.
Main duties of the job
Commissioning end to end care pathways, inclusive of care pathway redesign and service specification.
Leading, convening and coordinating a significant part of the ICB's strategic and operational planning process as a key member of the Leadership team in the Commissioning Directorate.
Securing market development and management and utilisation management approaches to deliver on the commissioning portfolio.
Leading and overseeing the contracting and payment mechanisms to secure delivery.
Effective monitoring and management of operational delivery and performance.
Ensuring the effective development of strategic commissioning capabilities within the Directorate and wider organisation.
About us
In 2025, the boards of NHS South East London ICB and NHS South West London ICB took the decision to enter into a clustering arrangement. This means that we share a chair, chief executive officer and an executive team, but we remain as separate, statutory organisations.
This also means that the ICBs will share some teams, who will be working across both organisations. Some teams will also be working across both geographical areas and integrated care systems.
This is a period of change for the NHS. The Government's 10 Year Health Plan presents an ambitious vision for a future NHS, with emphasis on the three 'shifts' - from hospital to community, from analogue to digital, and from cure to prevention - all underpinning the new neighbourhood health service.
ICBs are also changing, with a clear focus on strategic commissioning and delivering neighbourhood working. This is an exciting time to work within our ICBs. We currently have the opportunity both to shape the future of our clustered organisations, and to be part of a radical transformation of care for the local people and communities across south London.
And while our two ICBs are different in some ways, improving lives for the people we serve is at the core of what we are both here to do.
Please note as you can only be employed by one ICB, this job description refers to 'the 'ICB', but your role may cover both ICB organisations, and / or geographical areas.
