Job summary

We are seeking a Clinical Pharmacist to join our our well-established, friendly team of pharmacists and pharmacy technicians who provide clinical support to mental health units, community hospitals and community teams across Hampshire. This role is for 30 hrs per week.

The post is based at The Orchards Mental Health Unit at St James Hospital in Portsmouth and the successful applicant will provide regular clinical pharmacist support here and at the OPMH inpatient wards in Gosport War Memorial Hospital. Stoke-on-Trent NHS Talking Therapy Service is an innovative primary care service which involves partnership arrangements between North Staffordshire Combined Health Care (NSCHCT), Midland Partnership Foundation Trust (MPFT), and the Third Sector. We are looking to recruit a Cognitive Behavioural Therapist to provide a range of effective treatments for a broad range of common mental health problems in Primary Care, focusing on anxiety disorders and depression. Promoting excellence in the delivery of CBT, we are committed to continuing clinical training/development, supervision and support for all therapists in line with National Guidance and continue to strive as a high performing IAPT service. Main duties of the job

Have responsibility for specialist rehabilitation and therapy assessment, specialist intervention, care planning and review of individual clients.Delegate tasks to RSWs.

Case manage people through intensive short term rehabilitation programmes. Goal setting with clients to maximise independence, facilitate early hospital discharge, reduce reliance on care and reduce admissions to long term domiciliary care.

Work, as a lone practitioner and at times with colleagues, across a variety of locations within the community to meet the needs of a multi-cultural population in an inner city area.

Work as an autonomous practitioner complying with the standards set by the regulatory body (HCPC), professional body (CSP), government, Trust and department, thereby being professionally and legally responsible and accountable for all aspects of own work and to meet the requirements for professional registration.Work as part of a flexible multidisciplinary team, across traditional professional boundaries to meet the needs of clients using a holistic and client centred approach

To be responsible for practice, education, assessment, mentorship and preceptorship of students and new/junior members of staff.
